Does Kratom Show Up on a Drug Test?
Evidence-based answer reviewed against SAMHSA cutoffs and FDA assay documentation · Updated August 2026
Quick answer: No — not on any standard panel. Kratom is absent from routine 5-, 10-, and 12-panel employment drug tests. Specialized LC–MS tests for mitragynine exist (labs vary, often 1–100 ng/mL); not on any standard immunoassay panel.
Which panels screen for it
| Panel | Kratom status |
|---|---|
| 5-Panel | ❌ Not included |
| 10-Panel | ❌ Not included |
| 12-Panel | ❌ Not included |
| DOT 5-panel (safety-sensitive) | ❌ Not included |
The details that matter
Kratom is not on standard workplace panels. Its active alkaloids bind opioid receptors but are structurally unrelated to morphine, so opiate immunoassays do not react — a kratom user screens negative on a normal 5/10/12-panel cup.
Specialized liquid-chromatography testing can identify mitragynine and 7-hydroxymitragynine, but those tests are ordered mainly by pain clinics, researchers, or coroners, not HR departments.
Some 'legal high' warning systems now test for 7-OH specifically as concentrated extracts grow. If your program tests for novel psychoactive compounds, ask which expanded list applies.
State law matters: kratom is banned in a handful of states and municipalities regardless of testing practices.
If it IS detected, how long does it last?
| Test type | Typical Kratom detection window |
|---|---|
| Urine | occasional: ~3 days · regular: 5–7 days · heavy: up to ~9 days documented in chronic users |
| Saliva (Mouth Swab) | any: ~1–3 days (specialized) |
| Hair Follicles | up to 90 days (rarely offered) |
| Blood | any: ~2–3 days |
False-positive notes
- Kratom does not cross-react with standard opiate screens in validated assays; older studies suggested weak methadone-assay interference, resolved by confirmatory testing.
Remember: immunoassay screens produce artifacts; mass-spec confirmation and the MRO review are where legitimate explanations win.

How accurate are drug tests?
Lab-based immunoassay screens agree with confirmation testing ~97% of the time; confirmed GC–MS results are considered definitive. Home kits trade legal defensibility for convenience.
